The purpose of this research is to understand who participated in On The Table conversations as well as receive feedback from residents of northern Chautauqua as it pertains to the current and future state of northern Chautauqua County. 

This survey will take approximately 10 minutes to complete. Completion of this survey is voluntary, you may skip any question, and there are no right or wrong answers. Your answers are completely confidential and anonymous, no direct identifiers will be collected as part of this survey. The Northern Chautauqua Community Foundation and our On The Table partners may have records of the names and email addresses of some participants however none of this information is directly tied to your individual responses. Results of this study will be publicly available at www.nccfoundation.org/onthetable
